Presunaptic neuomuscular junc ( eaton Lambert)

Clue cells ( bact Vaginosis)

Lichen sclerosis ( mc cause of itching vulva esp in elder & should be confirmed by Biop but i'm not sure, could have been SCC if it was ulcerating & wouldn't go away)

Isn't in bacterial vaginosis the pH is 4 and 5 in trichomonas vaginalis.

Normal vaginal pH is <4.5

In both BV and Trich, it is ELEVATED >5.0
And to compare with Yeast Vaginitis, here the pH is normal (<4.5)

I have question related with No. 1.
I know that in this case, D is the answer....
my question is in paraneoplastic syndrome (A: muscle membrane) should we find always normal reflex? that's mean, decrese reflex is key word por E-L syndrome?

THE answer fot the first q is AAA . IT IS a case of myositis ( paraneoplastic disorder.). UW has a very similar question. .

for lambert syndrome, pt will have muscle weakness that improves on repetitive tasks... pt will feel better at the end of the day...


the second q, yes it is bac vaginitis. for Trichomoniasis, you will have more irritable s/s such as dyspareunia, and vulvar itching

SORRY, the answer for A is Lambert which has reduced or absent reflex. for myositis ( paraneoplastic disorder), the reflex is normal (per UW, just checked)

A Sympathetic Nervous System, this is Horner's Syndrome not Eaton Lambert. EL would manifest like a Myasthenia Gravis type presentation
B clue cells coz its a vaginosis, coz pH is >4
D coz LS usually presents with a itchy vagina

So for the second question...all we know is that she has a grey d/c which can be either trich or garderella. Also, the ph is >4.5 so it can still be either trich or gardnerella! Now, this is girl sexually active with girl as opposed to guy. So I would think this would be more likely to be gardnerella and thus clue cells as trich is more likely an STD between a girl and a guy.

This may be the worst rational ever to answer a q but it is all I got based on what they are giving us.

Good logic, just a few more points:
GRAY discharge and fishy odor is typical of BV (gardnerella vaginalis) and this is NOT typically an STD... It occurs when the pH is elevated, thus causing a change in the NORMAL flora (decrease lactobacillus and increase in gardnerella and increase in anaerobes)... However if a woman is sexually active with another woman who has BV, it's possible she could get it from her.

Frothy GREEN discharge is typical of Trichomonas vaginalis which IS transmitted sexually (third most common STD in the US!)
