Please answer and explain in brief.

Malingering ( having trial in 12 hrs & everything is normal)

Dementia alzh type ( if psudo dementia was there, i would have chose it, but i rememb UW saying, If inbetween ( Dem + Dep) & pseudo demntia isn't there, then Alzh Dementia is the ans)

Huntington D ( FH unknown & pt has chorea)

I am not sure about alzheimer one. for couple of reasons: 1. similar episode "successfully" treated by medication , and we know alzheimer is progressive degeneration. 2.psychomotor retardation and flat affect as the first thing 3.weightloss ...
And I don't have a perfect substitute. Parkinson's can have all these, but well they don't mention other physical aspects. I agree it is not pseudodementia, or multiinfarct . What do you think?

Dont u call it as major depressive disorder. IF dementia is associated with it then it is called pseudodementia. so what do u think about MDD as an answer?

for the second qustion cannot be alzeimers cos pt can date the symptoms to beginning 6 weeks ago. alzeimers pt can never date their symptoms it is an extremely insidious process.
cannot be pseudodementia cos pt doesnt appear concerned abt the problem, which is very typical of pts with pseudodementia.
that leaves us with multiinfarct and major dep
multiinfarct i think would have a little more neurological features other than a poor MMSE
also the history of 2 similar episoded in the past sucessfully treated makes me think of major deprsn as the answer
loss of appetite sleep and wt.etc also sems to support that diagnosis.
what do you gusy think?

I thought pseudodementia is the dementia caused by depression, isn't it?

Yes, No 2 is major depressive disorder.

1. Malingering
2. Multi Infarct Dementia. It has multiple episodes & in DAT it is the recent memory which is lost first & then the distant / long term memory.
3. Why not Wilson't disease ? It has all the components of wilson's disease.
Please correct me if I'm wrong.

1. Malingering.
2. MDD. Only this can be successfully treated. Once you have Alzhier's or strokes/infarcts causing dementia, you don't regain function.
3. Huntngton's.
