yes could be B . nonsexual transmission of the virus is possile from mom to the baby, so maybe type the virus in both mother and baby and if it doesnt match go for D.what strongly favors B is the absence of any other features suggesting a gonocoocal infection or trachomatis infectn, as also the absence of evidence of physical abuse, no tears etc. but could also be D since it is necessary to always maintain a high index of suspicion for child abuse.and again the option B says type mom not type baby too. ?????????? on the exam i would go with B, cos the question seems to be taking pains to point out that there isnt much of chance of abuse here.

Answer is D. 3 year old child should NOT be bleeding from her vagina! Plus condyloma accuminata on the perineal area! Highly suspect CHILD SEXUAL ABUSE! Remember, your patient is the Child, not the Mother! mom has a wart but no history of condyloma accuminata. Diagnose and treat the child. thus do cultures on the child first.
