A. proximal muscle weakness is due to myopathy asociated with cancer.
this is the case of Paraneoplastic syndrome.

D is not the answer because that is Lambert Eaton syndrome

eaton lambert syndrome, antibodies against presynaptic calcium channels.
smoker + lung lesion + wtloss = malignancy and
proximal muscle weakness= eaton lambert , therefore oat cell ca of lung.

It is D Eaton lambert syndrome NOT MYOPATHY, key in text
The major clinical finding is progressive weakness that does not usually involve the respiratory muscles and the muscles of face. In patients with affected ocular and respiratory muscles, the involvement is not as severe as myasthenia gravis. The proximal parts of the legs and arms are predominantly affected. Many patients have autonomic symptoms like dry mouth or impotence. Reflexes are usually reduced or absent. (KEY) (Which is not the case in the Paraneoplastic syndrome myopathy NORMAL reflexs).

i remember reading somewhere in UW, that in elderly patients it is somewhat normal to have loss of ankle reflexes, as it has no clinical significance, is that correct??

the ans is presynaptic neuromusc junc (Ca ch)

and yes in elderly its normal to loose a degree or 2 of deep tendon reflexes d t aging
