A 75 year old female has increasing kyphosis along with back pain. Her serum calcium is 9.8 mg/dL, phosphorus 3.8 mg/dL, and alkaline phosphatase 25 U/L. Her total serum protein is 5.5 g/dL with serum albumin 3.8 g/dL. Which of the following conditions is she most likely to have:
A) Multiple myeloma
B) Metastatic carcinoma
C) Paget's disease of bone
D) Osteoporosis
E) Rickets

total protein - albumin = globulin which should be more than 4 or 4.5 to make a diagnosis of multiple myeloma, I believe. I think its osteoporosis here...hmmm

Yes it cud be D but i chose MM on the basis that her Alk Phos is not tht high and co-relating to her increasing kyphosis. I have this theory that if ur Alk Phos is within normal Limits then its not ur bone pathology. i dont know whether i m right with this or not??

Any comments will be appreciated.

D is the answer,there were no expln for these q though doc 179 makes the very imp point.
