Should I send this form with the cover sheet to my LOR writer?

I just called the ECFMG. The anser is "no". they only need the cover sheet with the letter if the writer send it.
Also there is update of the cover sheet in late june. please use the new cover sheet which indicats IMG or AMG in the title. But if you used the old one, it will not cause any trouble.

Man, are you asking about ERAS document submission form or LOR request form? These are tow separate things...
