What is the Drug of Choice for the following Conditions in Pt w/ Park

1. Tremor & Mild Rigidity that do not interfere w/ daily tasks in an 71 y/o Pt

2. Tremor & Mild Rigidity that do not interfere w/ daily tasks in an 58 y/o Pt

3. Severe Bradycardia intefereing w/ daily task in an 71 y/o Pt

4. Severe Bradycardia intefereing w/ daily task in an 58 y/o Pt

5. Pt w/ Drug Induced Parks

6. Pt on L Dopa but Sx's persist

Age >60 yrs avoid selegiline, amantadine and anticholinergics.give sinemet (Carbidopa/levadopa)

50--59 yrs--Seligiline or dopamine agonist or both

<50---Seligiline, amantadine or anticholinergics, dont respond put on carbidopa/levadopa

Not responded to medical treatment---Deep Brain stimulation

In drug induced parkinsonism---stop the offnding drug

drugs that induce Parkins--->

1. Antipsychotics

2 Metaclopramide.

3. CO, Mg

the best ttt for those is to stop exposure if possible or change the drug

do we use anticholinergics for drug induced Park's?

For drug induced parkinsonism, the best treatment is to stop the offending drug or reduce the dose of neuroleptics, use of anticholinergics is associated with severe side effects particularly in elderly.
