sunil jawalkar , 32 years old man from india, comes to annual checkup. he does not have fever, nightsweats or cough. he denies sick contacts. he was given BCG as a child. his previous mantoux a year ago was 8 mm induration, and now it is 10 mm induration. what do you do?
a.start prophylaxis
b.dont start prophylaxis
c.send him to india
d.give BCG again
e.start him on 4 drug therapy

these are always bit -tricky, so do kindly explain, but I think it's (a).start prophylaxis ...

answer is a

start prophylaxis on anyone, any country, any age , if he was previously mantoux negative (2 years ago) and postive now. no matter, whether you are below 35 or above 35, or even 95, you will need prophylaxis. DONT FORGET THIS UNTIL YOU ARE DONE WITH STEP 2 AND 3!
