A 42 y.o. patient experiences muscle pain with his daily routine.The pain medication he takes irritates peptic ulcer.He also has a family history of coronary disease.Which of the following is indicated for pain management in this patient?

a.-Aspirin
b.-Celecoxib
c.-Acetaminophen
d.-Indomethacin
e.-Piroxicam

cuz cox 2 and NSAIDs agravate peptic ulcer disease and we can put this pt on dypiridamol or clopidogrel for anti platelet activity.

what about B.

celecoxib have no s/E. of gastric ulcer,,,,,,,

nope,with family history of coronary disease you dont want to give b.-Celecoxib and get sued

i forgot it totally.
actually clinical trial says other NSAIDS also increases the risk of heart dis.
so the best choice would be Acetaminophen

but I think in this case you dont give other NSAIDS becoz of peptic ulcerrolling eyes

but hw acitaminophen??? i does worsen peptic ulcer disease as well as no help fr cornary disease.....???

yup... it does not hav antiplatelet action....

Acetaminophen is the drug of choice because it works mostly in the cns while in the periphery it's mostly inactivated.

They are not asking for a drug to treat the patient's heart problem rather they are asking for a medication to relieve the patient's pain. so, the other choices should not be considered except for the contraindications due to the patient's family history.
