If you have watched the video, you should find this one easy (i think its an important fact too)

A diastolic murmur accompanied by an increased preload can be caused by

a. Mitral stenosis

b. Mitral regurgitation

c. Atherosclerosis

d. Aortic regurgitation

e. Tachycardia

1st : (C)

The magnitude of the cardiac output is regulated to maintain an adequate blood pressure and to deliver an adequate supply of oxygen to the tissues. In anemia, a greater cardiac output is required to supply oxygen to the tissues because the oxygencarrying capacity of the blood is reduced. In aortic regurgitation, the stroke volume will be increased. However, a portion of the blood ejected by the heart will return to the heart during diastole. Thus, the output delivered to the tissues does not increase despite the fact that the blood ejected by the heart has increased. In hypertension, third-degree heart block, and cardiac tamponade (decreased filling of the heart due to accumulation of fluid within the pericardium), cardiac output will be normal, or, if compensation is not possible, cardiac output will be reduced.

- i think this is important to remember -

2nd : (D)

A diastolic murmur occurs when blood leaks back into the ventricle through an incompetent pulmonic or aortic valve during diastole or travels through a stenotic mitral or tricuspid (AV) valve during ventricular filling. Preload is increased during aortic or pulmonic regurgitation because the blood retrogradely passing through the valve will be added to the blood entering through the AV valves. Preload is reduced when the AV valves are stenotic because less blood is able to enter the ventricles during diastole. Therefore, regurgitation of blood through an incompetent pulmonic or aortic valve will cause an increase in preload and a diastolic murmur.

Thats a +vety not a -vety. Remember that many of the Qs are made so you learn from them. There are some facts that arent covered in KPL LN but you find them in the Q Bank. Other facts youll find in the DVDs and so on. All the materials help each other.

though its tricky when the mcq is about a graphconfused i hate such kind of mcq

i realized something basic like acidosis & alkalosis can b tricky on step 1

partially compensated, not compensated ....lalalala but u know what gives me some relief when i read q book they say very intersting thing : usmle is not a test which indicate how good as physician u will b ,,,

its true i agree with this but u know step determine ur destiney...so iam ready 2 do whatever it takes i got very emotionalraised eyebrow

Yoga, I think the system based approach you are taking is a great idea. I think I'm gonna start a journal soon too and I may just be following what you do. I just need to finish my first read of all the material. I am doing my first read subject wise but I will do subsequent reads using the system based approach.

i would like 2 give sincere advices 4 those who r planning 2 start there prep

1. keep everything handy!! gather everything b4 u start!! believe me i takes time 2 print, copy, decide what 2 use/ what NOT 2 use..order dvd or whatever u r planning 2 use ahead of time

2. when u make a schedule make it realistic not too condensed, & remember u need break

becuz no 1 can study 24.7 & stay sane!

3. learn from others mistakes becuz u dont have time in ur life 2 commit them all!!

so this basically what we r doing in this forum

I will check on you . To not ppl foeget you, they will or will not. But to let them remember to pm or post here to check on you it will be better ... if you will come here at least 1 to 3 a week , to show you're still with us .

Happy or sad is best to celerate or sob with family & friends , so hiding will make it worse , more of insane ... especially if sad.

GL & come back here & stick with one journal only , let us know which one. Though we all love to check on you, support you , but in the long run, if you're running around the blocks, all your Journals & posts , that will tired ppl 7 they will lost of interest, lost of tracks of you, since all have been giving all their precious time even just go to movie for study break for you.
So Yoga , I might be harsh on you here say post stay in journal in order to let us check on you , keep in touch with you , but it's only for your own good, for your interest of to sure we all know you're good & ok , sane , happy & studying hard for school at school time & for usmle for summer or vacation or school breaks that not for school exm study break .
