20 year old male presents with idiopathic Hyperbilirubinemia.

I agree. It's Gilbert's. It's due to slightly low amounts of UDP glucorunyl transferase, which is the enzyme that conjugates the bilirubin in the liver. It's harmless and the classic scenario is a twenty something old student that has exams (stress) and presents with scleral icterus.
Other things that exacerbate it are: alcohol, infection, fever.
