A toddler is brought to the emergency department with burns on both his buttocks. The areas are moist, have blisters & are exquisetly painfull to touch. The mother explains that the child accidentally pulled a pot of boiling water over himself. When questioned, the mother states that she has just gone to the next room to ans the phone when this occured. Which of the following is the most appropriate next step in the managment? 1 Application of mafenide acetate to the burned areas 2. Early excision & grafting of burned areas 3. Education of the parents on accident prevention 4. Promt administration of fluid resuscitation 5. Referral for proper authoroties for suspectd child abuse

Very good question----mmmm---Not too sure! Child has second degree burns--(Blisters)--->Have to resuscitate the child first before referring to protective services to prevent the development of third degree-- Answer : 4?

4. always any kind of resuscitation first, then you'll have time to refer for child abuse.

thats why its a good one, 2nd degree on a small surface area doesn need fluids.. I was wandering about the pain & the blisters but I totally didn now that mafenide is only for deep burns ( 3rd degree, that will need debrid) kaplan says the answer is 5 leave the child in pain & just worry about reporting the parents ( totally dn't buy it) anyone can comment

I think it is against ethics to leave the child in pain.
