60 yrs old man who had carotid angiogram 2 wks ago comes today with fatigue and nausea. On his PE was found painless reddish blue mottling of skin of extremities. His medical problems are CAD, HTN x 10 yrs, DM x 11 yrs.
Lab: WBC 10,000 with 10% eosinophils
BUN 46 mg/dl
Creatine 3.0 mg/dl
C3 low
Urinalysis:
Protein +1
WBC many eosinophils
RBC 1-2 HPF
What is the most likely cause?
A) contrast nephropathy
B) cholesterol embolism
C) diabetic nephropathy
D) post strep GN
E) acute allergic interstitial nephropathy

Renal failure with Eosinophils in the urine, and mottling of skin extremities--> Atheroembolic disease.
