In pts with LBW or prematurity, should we perform a transfrontal US to screen for interventricular hge or there is no screening for that??

I think we don't have to screen for intra-ventricular haemorrhage since it can also occur during the process of delivery

Interventricular/ periventricular hemorrhage due to LBW or prematurity usually is asymptomatic with sudden unexplained ht drop and also can be presented with sudden and significant deterioration. I didn't hear about screen for IVH on neanate, and US is used when patient present symptoms to conform dx.

eventhough it is more common in this group, I think there is no screening done unless you suspect it.

ok here is the answer .intraventricular hge !.not common to occur after 32 weeks and emedicine says that imaging studies is not always indicated in apremature infants ! BLUEPRINTS SAYS WE HAVE TO SCREEN BY US FOR IVHGE in every neonates in 1st week of live if he is below 1500gm as 50%of them have IVHGE ..other wise not said in blue prints we do US when we suspect it !eg (forceps delivery in premature .breech presentation .istrumental delivery . associateed cephalohematoma .skull fractures .in all of these conditions we may do US.and ofcourse .if there are symptoms sugesesting of intracraial hge (pallor .anemia .loss of moros reflex .convulisons .acidosis.shock...etc)

screening is a must in preterms and is done at 1 week age (thats just to catch the maximum number of cases as 90% of IVH will occur in 1st week) if a child later develops hydrocephalus...u would never know the cause of hydrocephalus if u hadnt screened it cud be meningitis...IVH...congenial aqueductal stenosis etc... Also one can prognosticate like grade 1 IVH will have almost normal development(thats just germinal matrix bleed) but grade 4 (big Intraventricular Hemorrhage would have high mortality and morbidity) so even though there is not much to treat in IVH in preterms...one can prognosticate and document the Hemorrhage. Volpe and Papile are two Grading systems used for IVH but i shudnt go in to details of that... reference...still looking
