Ischemic versus Hemorrhagic stroke ATHEROSCLEROSIS OF iNT CAROTID a IS THE MOST COMMON CAUSE FOR ISCHEMIC STROKE whereas hypertension is the leading cause of hemorrhagic stroke. The first test to do is CT scan without contrast to rule out hemorrhage. TIA versus stroke The main difference is duration of symptoms. In TIAs the symptoms duration is less than 24 hrs .

FOR MANAGEMENT OF ISCHEMIC STROKE: if the duration of stroke is less than 3 hrs u can give thrombolytics like TPA but if > 3 hrs then trombolytics have little role.

There are also embolic CVA & thrombotic one In embolic , the most comman source is the heart, so Warfarin & clopidogrel r a must u should always investigate for the cause after the pt is stabilized, i.e. If embolic--> echo, If thrombotic--> Clotting studies, If Ischemic---> Duplex US on carotids, If hypert--> investigate the cause of sudden deterioration of bl pr( esp renal) ,if TIA u have to make a dupplex US on carotid to see the degree of obstruct, if > 70 % surgery is indicated, If < 70%, then only aspirin yes u can give tpa before 3 hrs Unless CI
