Bioguy: The UW q-bank is STOUT. It requires that you think in more than one direction at a time, AND to also know your basic pathology, disease associations, and physiology. For example:

A 40 year-old lady complains of edema and decreased peripheral sensations in her feet. She is started on the appropriate medications. Her renal pathology is similar to which of the following disease mechanisms.

- and the answer they want is say, podocyte foot process effacement. You were required to make the presumptive diagnosis of DM-2, know that the renal disease in DM-2 is primarily nephrotic in nature (look for albuminuria to confirm your diagnosis), and select the answer from the list that details the mechanism of a disease that also presents with a selective nephrotic picture (with primarily albuminuria).

So that makes this a fourth-order question:
1. Make the presumptive diagnosis
2. Know the pathophys of that diagnosis
3. Select a disease with a similar clinical picture
4. Know the pathophys of the disease that you've chosen.

They're not easy, but they teach you LOADS.

To illustrate the difference:

A Kaplan Question:
"The antidote for acetaminophen poisoning is __________."
(Answer: N-acetyl-cysteine)

A USMLEWorld Question:
"The antidote for acetaminophen poisoning has a chemical functional group that also accounts for its functionality in the treatment of _________."
(Answer: Cystic fibrosis. The sulfur-containing functional group both regenerates glutathione to detoxify acetaminophen, and also serves as a mucolytic by cleaving S-S bonds.)

If Kaplan wants to stay competitive, they're really going to have to elevate their game.

thanks guys.. i dont feel that bad now... and ya ur right... I did the BEhavioral and on my first pass got like 80% and was like wow.. then I went and did an embryo/histo mixed block and got a nice dose of reality back to 47%...!!
Im just at the point now, where I dont want to study anymore and I just want this process over and done with. I m afraid that im going to forget things now.. which might already be happening, my tempo is off.. what to do... what to do..

Try to enjoy the whole process, since you can't skip it... When you find something useful, said out loud: "wow, I may actually use this one day to save a life or make it better!", when it's boring: "I am glad I can forget all about it once I pass the test"...

Chances are you can picture yourself using the information, then it will make more sense and make you happier...wink

thats a nice way to look at it arlete!

I starting to get this feeling that this exam is one of those where I'll just never really feel comfortable and/or ready to give it. ! there's always something new to learn!

And you'll never know everything, and you don't need to know everything to pass with a good score, or to be a great doctor!

Just do the best you can, I bet that IS A LOT!wink
