A truck driver is involved in a serious accident and receives second- and third-degree burns over his body. he is placed in the burn unit and, on the twelfth day after his admission, develops a wound infection with a bluish- green exudate. Treatment with cloramphenicol and tetracycline is unsuccessful. A septicemia results and erythematous vesicles arise over the burned areas, which ultimately became hemorrhagic, necrotic, and ulcerated. What product of this causative agent is responsible for the formation of these lesions ?

A- capsule

B- elastase

C- endotoxin

D- pili

E- Pyocyanin

Since the tissue invasiveness of the organism is very limited, P. aeruginosa usually uses accidental ports of entry (burns, wounds, intravenous and urinary catheterization, surgical procedures, etc.) to gain access to its compromised host.

Several extracellular products (proteases, elastase, etc.) help the invasion and dissemination of P. aeruginosa. Most isolates of the species produce exotoxin A, which is induced under the conditions of iron limitation that characterize many animal tissues. The target of this toxin is one of the elongation factors in translation during protein synthesis. Strains incapable of producing exotoxin A have reduced virulence. A general discussion of virulence and disease caused by P. aeruginosa is presented in another section of the text .

Yes, elastase, the organism produces two elastases, LasA and LasB, which act synergically to degrade elastin as well as to inhibit neutrophil chemotaxis and function.

C- endotoxin << causes Septicemia

Endotoxin - it cases stimulation of macrophages to produce IL-1, IL-6, TNFalafa, which cause the manifestations of endotoxin shock.

Elastase - has a major role in the production of the necrotic skin lesions described here, better Ecthyma Gangrenosum,

Pyocyanin is the pigment that Pseudomonas aeruginosa produces to turn the colonies green. I believe it is the only organism that produces this pigment.

but they have asked about septecemia....has to be
ENDOTOXIN

have wait for the answer

Drduck, the question was about the lesions on the skin as the complicaton of septicemia - Ecthyma Gangrenosum, answer is D,

http://www.emedicine.com/derm/topic539.htm More info about it.

by the way GG, i think you meant to type B, not D, for the answer.

Sorry, my mistake, yes of course B- Elastase

well question is about the lesions and the answer is Elastase.....pyocyanin and endotoxin are also related to P. aeurigenosa but the question here is aboiut the lesion.not the septicemia and pyocyanin does not cause lesion.......
