A 15-year-old female is brought to your office by her parents complaining of absent secondary sexual characteristics and amenorrhea. Her medical history is unremarkable and she denies taking any medications. Examination reveals absent breasts, pubic and axillary hair. Vaginal examination could not be performed. Olfactory exam reveals an inability to identify different odors. Ultrasound shows a uterus and two ovaries; serum FSH level is 2 U/L (Normal is 4-30). Which of the following is the most likely karyotype to be found in this patient?
A) 45 XO
B) 45 YO
C) 46 XX
D) 46 XY
E) 47 XXY

My ans is D

yeah kalman syndrome ..(absent smell +absence GNRH) that is why FSH is very low and there is no development of secondary sexual character .as no (estrogen secretion by the ovary ).i think her karyotype is normal !.46 xx

The ans is C for sure, this is hypothalamic pit failure + anosmia

Next step is an MRI to exclude a brain tumor.

then Estrogen & progest ( OCP) for development of 2ry sexual ccc
