i have a pretty silly question abt how to go about the ccs cases... havent had any clinical experience so need someone to answer them..please help. For a STABLE patient in the OFFICE setting, with an apparently non-emergent condition, we order ROUTINE tests... now once the tests are ordered, should we 1. send the patient home (change the location)? 2. ... with an appointment scheduled after ALL the ordered test reports are available, or after the FIRST available test? i know that it all depends on the disease and presentation- but still, considering a totally stable patient with a very benign condition- say, alzheimer's disease, or fatigue... 3. what if the test results are to be reported on the same day ( day 1), but after 3-4 hours of the patient presentation? should the patient be sent home without reviewing the test results the same day? please answer and oblige.

Basically we do what we do in real life situation. For tests done in office itself like UA, Urine pregnancy test, Monospot, strep, finger stick glucose etc, we can interpret the results while patient is still there. Generaly for any test for whhich results are available in 30-40 minutes, we can wait. For all others we send the patinet home with appointment in 2-3 days. Sometimes we can call(schedule in CCS) the patient from home for an early appointment if test results are highly abnormal. Eg. blood sugar of 300 in an undiagnosed diabetic. There is no point in making the patient wait for any longer than 1 hour in the office if the patient is stable. I hope this helps.

hi holyman,egomez.I have posted this doubt in the forum but no response as yet.Can u guys help me. 1.In some cases the 5 min screen comes up as soon as the patient gets better and u re not able to shift the patient to wards or discharge him...is that alright? 2.in the giardiasis case once u reach the 5 min screen and patient is better what does one have to do ...do u cancel the antibiotics and other procedures.Will deleting previous orders mean that they are out of the patient management.....they say u can cancel orders and write fresh orders... 3.What exactly should be done after the 5 min screen appears.plz tell me It would be great if u could tell me what u would do after the 5 min screen appears for the cases in the usmle cd...giardiasis,ra,tension pneumothorax thank you
