A 2 weeks old male has been throwing up for the past 12 hrs. The vomitus is bilious and foul-smelling, no diarrhea. Pt is lethargic, pulse 160/min, BP <50 systolic, R 70/min. PE extremely dehydrated.
WBC normal
Htc normal
Na+ 120 mEq/L
K+ 5.1 mEq/L
Cl- 90 mEq/L
HCO3 20 mEq/L
Glucosa 60 mg/dL
pH 7.34
pO2 84mmHg
pCO2 34mmHg

What is the underlying cause?
1. Cystic Fibrosis
2. Galactose 1 phosphate uridyl transfererase deficiency
3. pyloric stenosis
4. salt wasting 21-OH deficiency
5. Group B strepto infection.

Complete Congenital Adrenal Hyperplasia---Salt wasting 21-oH deficiency--Most common

Hyperkalemia
Hyponatremia
Hypotension
Acidosis
Dehydration

Fits in correctly!

yeah i agree with sprint ! cannot be pylorric stenosis !!.it gives projactile NO BILOUS VOMITUS ,which is not the case here

IT is correct, you guys. this is classic question of congenital adrenal hyperplasia.
