Ok here's the deal, I got a doc from Canada to accept my request for an LOR after doing an externship. Now during this externship I got hands on clinical experience which he will mention in the LOR. The thing is that this doctor (among others there) trusted me so much that they even left me alone to question and examine patients because they had so much confidence in me. I was thinking that he may add this in, but for some reason I get the feeling that some US docs may see this as negligence on their part. Im really not sure on the protocol in the US as compared to Canada in this respect. What are your opinions?

Don't state you were not supervised. That would mean irresponsability from both you and the other doctor, at this point of your life. Do write about good communication skils, scientific knowledge, dependability, pontuality, going the extra mile for the patient, things like that!

I would be very very careful about what you mention in your LOR. One of my cousins who interviewed at a hospital stated that he took histories, and the program ended up not matching him only because they considered that practicing without a license. I would only state that you observed..

I M WORKING IN A SMALL CLINIC .IS THERE SOMETHING WRONG IF THE DOCTOR WRITES IN LOR THAT I TOOK HISTORIES AND DID EXAMINATIONS UNDER HIS SUPERVISION.IF YES HOW WILL HE MENTION THAT I HAVE HANDS ON CLINICAL EXPERIENCE.

nsg1000, how is it that many programs state that they prefer "hands on" clinical experience? and how am I even practicing without a license if Im not getting paid for it? being that its under a doctor's supervision is like it is with students and their teachers.

Don't say you were unsupervised, don't say you were "hands on" if it was supposed to be an observership. That means going against the rules and may be harmful for your image and of the hospital/doctor who was in charge. Practicing without a license has nothing to do with monetary gain. If you are doing a clerkship or internship, you have legal rights to practice Medicine under supervision, sometimes you even have to have insurance. If you are in an observership, that's illegal. If it happened, lucky you, somebody took a risk trusting you and signed your work, as if it was his. But don't mention it, don't get into those details thinking it's going to help you. It may backfire.

It was a planned externship, not an observship so by definition I should be able to have hands on experience. The letter will mention that I was always supervised. The rest of the letter will probably mention the qualities and traits I displayed.

thanks guys for yr reply. one thing more is it neessary to write that it was an internship, externship or clerkship or observership.because in the clinic my supervisor nor me said anything about it.i just asked him that i wanted to have some clinical experience he agreed and i didnt have any insurance,but the management person told me he did some HIPPA work and research for me to check i can do it or not.
