how does the metyrapone test help in hypercortisolism? can someone help me clear it up?

the Metyrapone test helps us differentiate in HYPOcortisolism not in HYPERCortisolism. For Hypercortisolism we do Dexa Suppression test.

Metyrapone test is used to Chk ACTH reserve of the Pituitary Gland. Metyrapone inhibits the 11-Hydroxylase which causes dec. production of Cortisol which further leads to Inc. ACTH production by Pituitary Gland and 11- Deoxycortisol in Adrenal Cland as its Proximal to the block in the Steriod pathway. In Hypocortisolism of Pituitary Type Neither ACTH nor 11-Deoxycortisol r increased while in Adrenal Insufficiency only ACTH gets inc.

thanx new_n_lost, but i have understood its role in identifying the level of defect in hypocortisolism. however goljan notes mentions it in hypercortisolism as well, thats something i couldnt get!

Ahan ..... Well u must be talking abt tht Table ( In His Spiral Binded Photocopied Notes) where he has differentiated the Cushing's Disease Etiology well take this info fit into that table and u will understand y he has put it there. Just to Explain the difference of Pituitary and Adrenal disease.
