Can any 1 explain, why in alpha thalassemia, HCT is decreased, when RBC is increased. ( HCT must also increse rt if RBC is increased) I hope some one will explain. gl

Goljan RR 2nd Ed. Pg 197 Section II B point 2 Changes in Thalassemia i) Hb and Hct r decreased ii) RBC count is increased ( Unknown Mechanism)

There is uniform microcytosis in thalassemia,RDW helps here diff from iron deficiency anaemia where RDW IS INCREASED.That means microcytosis is not uniform.That what I remember. So probably though the no is raised hct is less since volume is reduced.And Hct is total volume percentage of RBC'S ,or something like that.cant go beyond that.

I agree with new n lost... may be microcytosis is so much that Hct is decreased while the number is increased....but unknown mechanism is whats the latest consensus
