A mother brings her child ( not present ) , while asking for the CC she hands you over a paper with vaccination schedule on it what do we ask next.what do we counsel , since that perhaps might be the major part as there wont be a Physical exam.Can someone help me on the relevant Q's to ask and how to tackle the vaccination schedule ?

if mother gives u vaccination chart..... there willl b clearly written wat vaccines are given and wat are due .... at wat month so u can explain her that and if she has been regular abt her kids vaccines uptill now acknowledge it by saying i m glad to know that ur kids vaccines are up to date .....basically watever is written on chart tell her ... wat month is her next vaccine and all ...... imp abt routine health check up ..... point is just b very considerate and caring .....

thankyou for your time. Is there a standard format for a vaccination chart used everywhere in USA that may have a comment /conclusion box to it ?? But if it has to be so then it can be helpful.I havent seen one.Do inform me if there is sample somewhere or a format you have in mind , i dont know of any friends around so my only link is internet postings & feedback. I only know of the immunization schedule from CDA & P from Dept of Health & Human services, the one that i used for CK.But to interpret some other chart will need a quick eye.just dont want to be caught off guard for some trick
