Hello Again, So, I have just finished day 2 - I found it much more difficult than yesterday! The MCQs were pretty wierd. CCS was somewhat of a HORROR SHOW! For some reason, I got really nervous just before I started CCS and a started screwing up from the get-go. All my cases ended early - some of them the patient did not feel better and was still in pain even though I pumped them with morphine - Some of the cases were directly from USMLEWORLD CCS cases - others were not. I forgot to schedule follow-ups for some patients - ahhhhhh. Im really hoping my MCQs will help clear me through this. The CCS cases I got were pretty easy - I was just so nervous that I forgot a whole bunch of things. For example, this one patient I had complained of leg and thigh pain with walking up stairs and other exertion - intermittant claudication right - well, at the end the patient still was in pain, I forgot the name of the medication (cilostazol or whatever), and I went totally blank and couldnt think of the name of his condition - PERIPHERAL VASCULAR DISEASE - hahahaha. When it asked me for diagnosis I wrote "atherosclerotic disease with intermittant claudication of lower extremity" - oops. Now - lets see if I pass BTW - my cases were: 1. Fe Deficiency Anemia 2. Gastric Ulcer 3. Testicular Torsion 4. Peripheral Vascular Disease 5. Ovarian Torsion 6. Acute Diverticulitis 7. Dehydration 8. Opiate Intoxication 9. New Onset Diabetes Mellitus Pretty easy cases - Im just a goof!

hi bwaliamd I just took my exam, and my experience was so similar to yours , i am not really worried about mcq, but my ccs went terrible, i think i messed them up almost all of them, but anyway I would like to know how you manage the testicular torsion case, because i got it too, and i did diagnose it but there was not way i could treat him, the program did not have the word orquiectomy, because in my case the testicle by scan was infarcted already, and the boy was in so much pain even though i put him in morphine, of course he was in pain, but the case ended and i could not do surgery, so please tell me how you did it. gl with the score, thanks

bwaliamd, Thanks for sharing your experience with the forum. Just to let you know that according to First Aid, the things we write in final diagnosis or the reason for consultation, do not count towards our score. And it makes sense cause the software will not be able to interpret what we write in those sections. Only the items we choose from search results by clerk can be understood by the scoring software as they are already in the system. So if you managed the case appropriately, it does not matter what diagnosis you write. I guess we all have this uncertainity over our performance after the exam and we all miss something or the other that we remember right after the exam is over .

Hello, It has been a month since I have taken USMLE Step 3 so I cant remember exactly what I did for each CCS case although for the testicular torsion case I remember that I asked for a surgical consult after receiving testicular ultrasound results and the patient was taken for surgery right away. Hence, the case ended. It has been exactly four weeks since I took the exam. I have been fine up until now - meaning I have not been worrying about passing or failing HOWEVER, the nightmares have started - ahhhhhh. I find myself dreaming of passing some nights and failing other nights. Last night, I dreamed I got a 74! My score should be reported any day now. Congrats holy man on your fantastic score
