A 25-year-old white male presents with sudden onset of pain, redness and swelling of the first metatarsophalangeal joint. He also complains of low-grade fever not associated with rigors and chills. He is a non-smoker and non-alcoholic. He has history of hypothyroidism and is on levothyroxine. Examination shows redness, swelling and erythema of first metatarsophalangeal joint.
