A 32 yo man with AIDS is receiving mechanical ventilation for a respiratory failure induced by a PCP. Use of TMP-SMX is stopped after 4 days because of severe skin rash. Which one of the following would you initiate at this time?

A. Atovaquone

B. Primaquine + clindamycin

C. Pentamidine

D. Dapsone

E. Metronidazole

For therapy of PCP: TMP-SMZ, Pentamidine, or Steroids

For prophylaxis of PCP: TMP-SMZ, Dapsone, Atovaquone, or Aerosolized pentamidine (last resort).

I am sorry guys... Pentamidine alone is not the drug of choice in case of sulfa drugs allergy... Try again! Tricky q, I got it wrong too...

For therapy of PCP it is TMP-SMZ first choice... OR Pentamidine. If allergic to TMP-SMZ, then Dapsone.

(Kap only mentions Pentamidine--- just curious to the source which says Pentamidine-Sulfadyazine???) I only know about Pyrimethamine-Sulfadyazine for Toxoplasmosis treatment.

Almost all options would be correct, but I guess the 2nd choice in this case would be B (clindamycin and primaquine).

this is what i thought was the correct treatment and prophylaxis for PCP... TMP/SMZ is first choice... if allergic to TMP/SMZ then use pentamidine
dapsone and atovoquone are used for prophylaxis only...

Even Dapsone is a sulfone drug, I think it a person is allergic to sulfamethoxazole, we must not give dapsone to him. what do you say guys.

The right answer is B. The q comes from the mayo clinic review for ABIM certification.

I guess it is the usual explanation: a mix of efficacy and safety profile... Not that some of the other options are not correct, but B is the best one. I don't think this is the actual profile of questions in the USMLE, because there usually one is clearly the best option.

WOW DrVirgo I just realized my TERRIBLE mistake! Yesterday I was in a hurry and didn't read your previous post. I clearly consfused Pentamidine and Pyrimetamine in my reasoning! In the expalnation it doesn't say anything about sulfadyazine, it was my WRONG confusion.

So guys Pentamidine is not used with sulfadyazine, pyrimethamine is. For PCP in sulfa allergic patients the best alternative is The combination Primaquine + clindamycin; Pentamidine is less effective than this combination (this what they say on explanation, they don't go in details). Anyway I guess that if not given the choice Primaquuine + clinda we should answer Pentamidine

I am really sorry for what happend, i edited the previous post in order not to confuse other readers
