Next goal, finish up a quick revision of preventive medicine and poisoning and do the UW questions again (I want to finish the 38% UW remaining/second round because i feel that i didn't master UW yet, i feel that i need to learn more from UW)

ok done with preventive medicine, in the second time revision i definetely picked up some extra info but still making silly mistakes for instance:
  • Mammography: Starting after 40, then every 1 -2 years until 70
  • Influenza in close contact to 0 to 59 months old (keep forgetting this indication)
  • In diabetics give yearly influenza and pneumococcus every 5 years
  • HPV for 9 to 26 yo F

Now onto poisoning

YOU are doing great Ivonne. Good luck! You work very hard.

  #440

Thanks anuba, just trying.

Again picking up extra info from UW for instance i keep making mistakes about diverticulosis versus angiodysplasia which are the most common cause of painless GI bleeding in the elderly but if the question gives hints as aortic stenosis or renal failure and a normal sigmoidoscopy lol the diagnosis is angiodysplasia. There is a well defined association between aortic stenosis and angiodysplasia.

When a patient has GERD and alarm symptoms:
  • Nausea/vomiting
  • Weight loss, melena, anemia
  • prolonged disease >= 1 -2 years
  • Not response to proton inhibitors

Proceed to do an upper endoscopy and skip an esophagogram if patient doesn't have dysphagia.
  1. Lactose intolerance has a positive hydrogen breath test
  2. Irritable bowel syndrome may present with mucus in the stool
  3. Folic acid can be depleted in 4 to 6 months while B12 stores last 3 to 4 years.
  4. Bacterial overgrowth/malabsorption syndrome/H/O surgery
  5. Anorexia /digoxin toxicity/exacerbate by verapamil Now onto Hepatology...

  • Disk herniation without cauda equina syndrome could be managed conservatively for 4 to 6 weeks with early mobilization and NSAIDs but if neurologic complications come up or pain persists then it is time to indicate MRI.
