i find performing an anterior chest exam in a female SP particularly challenging...do we have female SP's in which we will have to do the CVS or RS exam. i am not sure how to do the draping in such a situation. how would we do a PMI in a female...ask her to lift her breast herself or do it with the back of our hand after letting her know what yu are doing ?? during the RS and CVS exam can we lower the gown upto her upper abdomen( i am assuming she will be wearing something underneath for sure) or it would still be considered inappropriate to lower the gown to her upper abdominal level even if she is wearing something underneath ?? otherwise it would be difficult to perform these examinations like the anterior chest expansion, anterior chest percussion, locating PMI, asking the patient to turn left while auscultating the mitral area. thanks in advance guyz...yu all have been truly helpful with whatever trivial confusions i have and ultimately make it clear for me on what to do the exam day. thanks.

U can expose her after taking her permission, no problems with that, but don't play around with the other clothings

check out this thread http://www.prep4usmle.com/forum/thread/54561 its ok to assess pmi in a female... inform the patient what you want to do... get permission... ask the patient to lift her breast if its in the way... don't remove the bra... good luck!

thanks a lot to both of you. i was really wondering if i should even ask to lower the gown to upper abd in a female SP...i was wondering if that would be inappropriate to ask...but now i will feel more confident in carrying out this examination. thanks again guyz.

yes, it means "with" but check in the first aid or usmle website if you can use it before the exam so that yu dont lose out for the examiner not been able to understand what it means.

Hey guys, It's used in usmle PN 2 nd one like mother C NIDm and brother C some heart prob, I couldn't do that while typing .Now does that means mother has and brother has so and so disease?/ Thanks all!

Never ask to remove the bra or move the breast by your hands on top of bra , that's the rules, that is why Breast , rectal exam are not allowed. Ask the pt to lift the breast byself. Rock, check , talk with any has already done CS in your city or town. Check at University, medical or local public library , alot are studying usmle includind CS. Alot IMG are doing or had passed the CS here . Key is practice with the one passed already . GL rock .

 , NO , not at CS exm . But yes, when you're DOC here depends on your specility , pt female chest can be examine by Dr. But be very careful, here full with law suit even after you have grand children.

thanks a lot shwehope. i really appreciate it. presently i am studying with a friend who is also yet to take her exam sometime in mid august. we talk to her sister who is now doing residency.most of the people i know who have passed CS are now in US so i guess i wont be able to do that but i hope i can talk to you all here and get all the advice i need and i do get it always.thanks again shwehope and good luck to you too. what i am going to do is...explain to her what i need to do and what i will examine and why it is imp and then i will pull her gown down upto upper abdomen and after checking other areas(aortic and pulmonary and tricuspid) i will ask her to examine the mitral area and also ask her to llift her beast herself so i can examine her and taking all care not to touh her breast. i will also auscultate for breath sounds while she is in the same position. i hope that is ok.
