I am an very old IMG( graduated in 95), staying in usa on dependent visa till 2009. Actually, I love clinical work( got IM speciality and many clinical experiences from home country) ,however it maybe very touch to get into residentcy with my situation( old graduation, visa need). I have received fellowship in Public health for Phd program but my heart still go with usmle( studying usmle for 4 months). Anyone please let me know if any chance for me to get into recidency ? Please give me advice: USMLE or Phd ? Any ideal is highly appreciated .

Follow your heart. A PhD is a lot of work if you don't really want to do it. Being an old grad is not such a big deal. I think it is possible to get a spot. Maybe difficult, but if that is what you want to do, then go for it!

if i were in your situation, i would take the PhD and while im doing in it i would study for the USMLE steps and after finishing the phd i would apply to im

U can do both, finish ur steps in PhD and then go for residency, this way u wud have some pubications and experience under ur belt as well

I was faced with the same Q 4 years ago-I decided to do my PHD but now going back to MLE, I feel it was not necessary- At some point you will have to decide on what you want- A PhD is a looong time committment and I would think a lot before getting into it- look at your time line- On average it can take upto 5 years for the PhD and then you Start your residency-which will last 3+years- You will be done with your studying only after another 10 years! But if you want to stay in Academics- become a Professor or something maybe its worth taking the risk -If your only reason for doing a PhD is to get a residency- then Definitely dont do it- you are probably much better off getting some clinical experience at the place you want to do your residency and making an impression there in a few months rather than spending years out of touch with Clinical Medicine
