A 23 yo presented with svere anemia and polyarthralgias. There was a history of a febrile episode. Bone marrow biopsy finding shows RBC hypoplasia.. What is the treatment of choice? A. observation B. ganciclovir C. Ciprofloxacin D. Immunoglobulin therapy E. Plasmapheresis

D. Although we'll never encounter a Questions with as little information as this question, looks like Parvo B19. We can't answer this properly because we don't know how "severe" the anemia is? If it's below Hg 8-10, then the answer would be to transfuse. If this patient is immuno-compromised, then he should receive Ig therapy. Since only "Ig therapy" is an answer, i go with D. (according to 2005 CDC guidelines)

observation. parvovirus infection, i think all the other treatment options are ineffective.

yes,we can give immunoglobulins to immunocompromised individuals.

Well i was thinking to just observe at first... but the questions says "Severe anemia", without giving us numbers...? So it's hard to assess whether he/she'll need transfusion or not.

Agree with youngdoc...Buy the best option available is observation --------They didn't mention about blood transfusion so let's wait!

The right answer is Immuneglobulin. It's a case of parvovirus infection and the treatment of choice is Ig.
