A 74 yo man presents with fatigue, weight loss and dizziness when standing. On further questioning he describes symptoms of a sensorimotor peripheral neuropathy. The physical exammreveals 2+ edema of lower limbs. Serum creatinine is 3 mg/dl. Urinalysis showed proteinuria. What is the next best test?

A. CT of abdomen

B. Bone marrow examination

C. Fat aspirate with congo red stain

D. Peripheral nerve biopsy

E. Small bowel radiography

tricky question, i am suspecting amyloidosis in this old fellow which can cause nephrotic syndrome and peripheral neuropathy.

so i will go with C.

Yes....Amyloid induced Nephrotic syndrome which could explaint he constitututional symptoms also!

i am also thinking amyloidosis. so i will go for fat aspirate for fat aspirate with congo red stain
