I was at a workshop back in the early 90's. They said that anyone having diarrhea for over one month's duration should be checked for O&P - specifically Giardia.

Our laboratory stopped doing full O&P examinations on April 1. We have substituted a direct antigen test for Giardia and Cryptosporidium. It checks for both on one sample. We cancel the O&P with a comment that the GICR test will be substituted, and to call our extension if a full O&P is desired based on travel history. We get virtually NO requests for further workup. This was done because we are in the midwest and the only positives we had for the entire year before this change were for Giardia.

Travel history is very important. We did have a request for a full O&P on a child that was adopted from overseas, and she was positive. Check your lab's policies and question the patient thoroughly to see if they really do need a full O&P workup. Chances are the GICR will do just fine. Document on the requisition any travel history that the lab should know about. This helps immensely when we do come across positives. The country of origin helps to know what to look for.

PLEASE don't be the docs that order everything under the sun on stools that are so hard they rattle around in the container. We get enough of these to wonder what these docs could possibly be thinking.......part of the problem could be the patient, however, who doesn't know the difference between diarrhea and formed stools.
