don't have the exact pathophysio - but it has something to do with an dysfunctional liver in addition to fetal rbc hemolysis...

It is called Peterson's hypothesis.

++Maternal Glucose ==> ++ Fetal Glucose ==> ++ Fetal Insulin ==> fetal hypogycemia & acts as GH.

also insulin decrease PG levels leading to RDS.
