HI bbb i have seen you helping alot of people here so i thought maybe you are the best person who can clear a few doubts. 1.There seems to be a general consensus among IMG's that they absolutely need USLOR and experience without which they donot exist. 2.EVEN among the USLOR's, letter from AMerican doctor is better than a letter from someone who belong to your native country but is practicing in US. 3.USCE in a pvt clinic is worthless. All USCE should be in hospitals and observerships donot count. 4.You can get filtered out by the software even if you are just 6 yrs away from graduation rather than 5 which is a cut off. I know the answers will be different for each an every program but i just want to know your views. I know i have asked alot of questions, this is not urgent. Take your time. Thank you,

1. Untrue. Many high quality programs take people every year that do not require USCE. 2. A good strong LOR doesn't matter who is comes from. US or overseas. Again, see answer above. 3. Since my program doesn't require USCE, I can't comment on this one. It makes sense that if you are applying for a program that does that they would want it to be in an educational setting, not just hanging out in a clinic - but every PD is going to have it's own ideals. 4. If the cut off is five and the program sticks to it, then 6 years will be cut out. Is it fair? Not really, but when a program receives 4000, 5000, 6000 applications a year, we have to start somewhere. My program has a five year cut off, we filter at 7 years to catch persons who may have done a residency in their home country - that is valuable experience. Good luck

Guys keep in mind that BBB is just one program director, and can't speak for them all. I don't mean to argue with BBB or anything, but I'm just saying this because i know for a fact that some PD's don't even look at LOR's that are not from US. physicians. I've been told that personally by a few Internal Medicine PD's.

Wow, thanks Young_doc - you just promoted me to a program director!! Gotta love that! (Very tongue in cheek, and thanks for your answers to the questions - everyone has different experiences that I can't comment on)

Well at this point there is no one who would say DOnt get USCE or USLOR isnt important. But thats not the issue here. The fact is most of us will be unable to find anything regardless of how much we try. Some of us will be lucky if we find some Pvt Physician who would let us observe. Honestly observerships at university hospitals are nearly impossible now (Without Contacts). So what do we do? whats the solution? I'd say do whatever you can find, keep trying but dont let these negative rumor mill destroy your spirit, mood for the next match. I found a US physician who agreed to give me observership at his clinic next month. My friend my so-called well wisher said to me "Well dont be so happy, his LOr will have no weight, your observing him will be a waste of time so DONT GO" Now i can understand that maybe this observership at a pvt clinic wouldnt weigh so much, but why should i leave it, stay home DONT GO?? You get the idea, its tough out there, people will bring you down, spread rumours. After all we are each others competition. Dont let negative scary thoughts ruin your mood and depress you. Goodluck.

Thanks for your post, Sarika! Very well said. After reading the forum, I was on a fence about the observership in a private clinic. But I guess it's better than nothing. I was told they recently adopted a law that everybody in a hospital should have malpractice insurance including those who do observership. I don't know whether it's true or not, but I had 'contact' in the hospital, and it didn't help.
