Hi, I'm reading physio kaplan and it's not doing a great job explaining the 2 types of cushings.. could someone describe the 2 ? thanks so much, pat

at least i think there are 2... 1 in the pituitary and the other in the adrenals?

Pituitary Cushing's is due to a Pituitary Adenoma which hypersecretes ACTH thus raising the Cortisol levels. The Cortisol Production can be suppressed by a High Dose Dexamethasone test Adrenal Cushings is due to Adrenal Adenoma producing Cortisol thus giving dec levels of ACTH but High levels of Cortisol. Can not be Suppressed by High Dose Dexa Test. The Significance of the High Dose Dexa Test is that it can differentiate bw the Pituitary and Ectopic Adenoma as both secrete ACTH. Both Present with High Levels of ACTH and Cortisol , High Dose Dexa can Supress the Pituitary production of ACTH while it cant in an Ectopic Adenoma cos the Ectopic tissue wont have the Feedback Mechanism incorporated in it.
