what is the treatment for HIV+ pt with cyptosporidosis ?

Cryptosporidiosis resolves within 14 days in HIV-infected patients with CD4 counts above 200 cells/mm3, no treatment is required for these patients.
For patients with CD4 counts less than 200 CD4 cells/mm3, paromomycin (Humatin®), a nonabsorbable aminoglycoside, is the treatment of choice.(23-25) The recommended dosage is 500 mg q.i.d. for 14 to 28 days. Chronic maintenance therapy is often required for patients with heavy infestations, those with CD4 counts below 50 cells/mm3, and those who relapse after initial treatment.
Azithromycin (Zithromax®) has shown some clinical activity against cryptosporidial enteritis and can be tried if paromomycin fails
Supportive measures are essential for patients with intractable diarrhea. These measures include both prescription and over-the-counter antimotility agents, antiemetics, narcotics, octreotide and enteral replacement of caloric, electrolyte, and fluid losses .

*There are no approved treatments for cryptosporidiosis. The drug paromomycin (Humatin) has commonly been used as a treatment, but a recent study found that the drug did not work.

* A very promising new treatment is the drug nitazoxanide (NTZ).

*Azithromycin (Zithromax) is another possible treatment.

*symptomatic treatement : Immodium, octreotides or opiates for the diarrhea. Hyperimmune bovine collostrum is being tested as a means to increase the body's production of natural immunoglobulins used in fighting crypto. Total parenteral nutrition, or intravenous feeding is sometimes recommended to replace nutrients.
