classical presentation of lead poisoning in adults.

Any patient with CRF and GOUT, always suspect lead poisoning.

He is having classical symptoms like abd pain,senory disturbances,memory loss.

the 'homemade' whiskey part is another BIG clue...as well as the low MCV (Lead affects the last step of Heme-synthesis...)

how does lead poisoning cause gout?

i think the gout is due to the alcohol and is meant to be a distractor

More than 50% of the patients with lead nephropathy manifest saturnine gout. Even though hyperuricemia is universal with renal insufficiency, gout is rare unless the patient has underlying lead nephropathy.

i must admit it is a tough question. It has all features of Acute intermittent porphyria - 1) abdominal pain, (2) psychiatric symptoms and (3) peripheral neuropathies

Interestingly both lead and AIP cause motor neuropathy and this patient has sensory signs

Presence of renal failure i think is more indicative of Lead poisoning
