either irritative or obstructive the intial tests are urine analysis and Cr
then?.......(this according to UW)
they say also old patient with irritative voiding symptoms and you did urine analysis and it was free and cultre free (although they said culture not done if normal urine analysis!) you must rule out bladder cancer by cystoscopy and cytology , and in the cases they always did before rectal exam and expressing and see WBC and culture for secretion , they they write statment at end of explanation , when patient old with irritative voiding and free urnine analysis and culture u have to rule out cancer !!!
i got confused , can any one put a right approach to all that what first . rectal exam or urine analysis and Cr , and what next prostate expressing and see WBC and culture or cystoscopy ???!

Of course u will always do a Physical Initially
then
Initial - Cr and UA
-If UA is + --> Next step Urine Culture
-If Negtaive --> r/o Infection
US/IVP are indicated if there are Structural complications that lead to recurrent UTI's or Complications of BPH. Indications are Cancer, Renal Insuff, Hematuria, Stones
pls correct or add

but still what will be our approach for some one come with symptoms of irritative voiding (say prostatitis) first rectal exam then Urine analysis and Cr then>>>>>??? expressing prostate for analysis and culture or rule out cancer bladder by cystoscopy?

ok if somone where to come in with typical Sx's of prostitis then
1st Creatine
2nd UA
3rd In all Cases of prostitis u can do Prostatic Massage to get secretions for Culture, except in Acute Prostatis u do a midstream Urine sample for culture b/c of the risk of septicemia
Rememeber the P/E, then Labs such as Crea/BUN... UA are always the same Initially, what follows after that will depend on how the case is presented. You have to go thry all the UW question in Kidney and read ALL explantions so that u don't miss any info
