wat we can do is while introducing ourselves we can just add up a line saying i will b taking few notes while asking questions to u , is that ok with u ??? and patient wil say yes ....

but make sure when ever u asking question look at SP ... make eye contact ... i guess it goes for everyone, why SP or patient ... wont it look rude if some one asking question to u, without looking at u ... unless its a telephonic conversation?????

and if u can scribble while looking at him and nodding to wat ever he/she says in legible handwriting its fine too i guess...

well i guess u will ask this question in relevant cases only ... like psy cases , drugs abuse , child with behavioural problem ... so as there is present history i dont think so it will sound rude to ask same as past history....

not sure though

From what i gather here , Explaing beforehand seems to b the key.I'll be taking notes, is that okay? .. Good eye contact time esp when asking a question as is said above is must for interpersonal & communication skills , not forgetting its CS actually.
Either writing down the pnemonic before or keeping it posted somewhere in forebrain, quick to recall, keeping that in view, asking questions and noting only positives.HOPI is relevant positives.Irrelevant negs wont carry marks & i lose time when its time managment.
