What is the treatment for both and how do they vary??

NSIM in DKA ?

1 ) Normal Saline + INSULIN

2 ) Replace K if normal or low but DO NOT give K if greater than 6 mEq/L

3 ) Do not usually give bicarbone for Board Purpuse !

4 ) Look for source of infections ( MC : UTI ....E.Coli )

NSIM of HONK ?


1 ) NORMAL SALINE

2 ) Insulin

3 ) Almost the same as above

The way I remember it on boards is :

DKA .................INSULIN , INSULIN , INSULIN , INSULIN + FLUIDS

HONK ...............FLUID , FLUID , FLUID , FLUID , FLUID + INSULIN

bicarb administ is contraversial but UW says if the PH is>7.2 it should be given
don't forget to monitor the glucose level as it may drop to below normal with ketones still +ve or acidosis +ve so u should continue insulin but add glucose
