In One question it states that you do Blood Cultures before giving IV empiric Abs and in another question is says 1st empirix IV Ab's then Blood Cultures

which is it??

I think you should give IV antibiotics, and not wait for results of blood cultures to start ABs...

Any other thoughts anyone?

Guys !

I think the point that UW is trying to make is :

Pyelonephritis can lead to SEVERE GRAM - SEPSIS and ABs RESISTANCE so the NSIM should be :

1 ) do blood culture

2 ) start empiric antibiotics ( But they did not say that you have to wait for the result of culture before starting antibiotics )

3 ) once you have the result of culture , you can give the appropriate antibiotic according to..........

As a matter of fact , When I had this question wrong in UW , I had the same problem digesting it too so I checked CMDT and they confirm it ( blood culture first and then empiric ABs )

I think it 's pretty apropriate guys !

I agree with doc_clotaire,
you start with empiric AB, and then adjust the protocol based on the microbio culture results/sensitivity.

yes agreed with doc clotaire, when u put the iv line, 1st take blood for cultures, then give the i.v antibiotics, till the results of the cultures re out, and then give the apropriate antibiotics as needed
