Whitish lesion over the vulva,prescribed itraconazole.lesion did not subside.skin overlying looked white thin parchment like. Diagnosis? Candidiasis, lichen sclerosus, leukoplakia Do both lichen and leukoplakia appear white? How do we tell them apart aside from the "thin parchment" or is that it? thanx

its very easy to differentiat between the two........... in the Q its given that.....skin is thin parchment like....so if they say thin.......first thing come to ur mind is LICHEN SCLEROSIS. LICH SCLE......lesions are particularly, small, subtle in form of white spots. The involved areas are usually slightly shiny and smooth. As time goes on, the spots develop into bigger patches, and the skin surface becomes thinned and crinkled. As a result, the skin tears easily, and bright red or purple discoloration from bleeding inside the skin is common. LEUCOPLA.......Leukoplakia is a condition in which thickened, white patches form on gums, on the inside of your cheeks and sometimes on your tongue. These patches can't easily be scraped off....mark that word...thickened. so things are now clear...... answer is LICH SCLE....

Leukoplakia is now termed as Lichen Simplex or Non Neoplastic Squamous Hyperplasia of Vulva and is thickened White Patches of Epithelial hyperplasia.

This condition affects mainly women above 50 years but can affect any age group including children. The lesion appears as white, atrophic, "parchment paper-like" or crinkled paper patches of the vulvar skin, occasionally accompanied by marked contracture with narrowing of the introitus (hence the old term kraurosis vulvae).-lICHEN SCLEROSIS Vulvar leukoplakia is caused by vulvar irritation. It produces hyperkeratosis (excessive surface keratin) and creates a white, plaque-like area that is often strikingly white even without acetic acid application. While leukoplakia itself is a benign process, it may cover or be associated with an underlying VIN or carcinoma, therefore biopsy is prudent to rule out a more ominous disorder.
