Most patients with Paget’s are actually asymptomatic, and need no treatment. Only treat those with symptoms, or when it’s the weight-bearing bones that are affected.. treat with bisphosphonates.

UW says indication for treatment of Paget disease:- 1.) bone pain 2.) hypercalcemia/immobilisation 3.) neurological deficits 4.) high output cardiac failure 5.) preparation for ortho surgery

Paget D is mostly asymp.. If discovered coincidently tt depends on ---> age < 30, Hypercalcemia, affection of weight bearing bone, high output cardiac failure & prep for sugery (only if there is hypercalcemia). then the next step would be biphosphonates in case on an acute present, the 1st step is normal saline infusion, then frusemide & then mentainance on biphosphonates
