1. Vignette of tuboovarian abscess—cervical swab demonstrates gram –ve intracellular diplococci.laparoscopy done and drained, sample obtained was examined: gram negative rods also found with diplococci. What organism is it most likely: bacteroides, Chlamydia, myco tbc, treponema 2. Mechanism of vancomycin resistance: plasmid mediated,chromosomal mediated, substitution of AA in a polypeptide?

1. N. Gonorrhea and Chlamydia inf ( they both co-exist) but gram Negative Rods indicate Bacteriodes. 2. substitution of AA in a polypeptide its the replacement of D-Ala by D-Lactate in the bacteria terminal chain.

its abscess buddy...abscess in an area where OXYGEN is not likely to reach n if it reaches it is taken up by neisseria anyway... the point is bacteroides ...the most common bacteria in COLON.... also present in 60% of women as a normal flora... soooo....bacteroides...

the other organism likely to be there should be microaerophlic streptococcie... anyway... chlamydia with neisseria usually CO EXIST...but abscess formation isnt a featuer of chlamydia....

vanc resistance isnt HIGH LEVEL as yet so its plasmid mediated...easy to rmember
