MULTIPLE MYELOMA is due to clonal proliferation of neoplastic PLASMA CELLS in the bonemarrow associated with MULTIFOCAL LYTIC lesions throughout the skeleton system.
5TH AND 6TH decade of life.
 "M component is IgG."
MC presentation is BONE PAIN and PATHOLOGICAL FRACTURES,MC affected is the VERTEBRAL COLUMN.
other imp features are the" MYELOMA NEPHROSIS' interstitial infiltrates of abnormal plasma cells and protenaceous casts made of bence jones proteins within the DCT and CT and HYPERCALCEMIA  metastatic calcification.

WALDERSTORMS MACROGLOBULINEMIA also involves the B cells that are sufficiently diff to produce immunoglobulins but not enough to look like plasma cells.
6TH AND 7TH decade of life.
 "M component is IgM '"
the neoplastic B cells diffusely infiltrate the LYMPHOID ORGANS including the bonemarrow, lymph nodes and spleen.
The macroglobulin forms large aggregates and increases the viscosity of blood  HYPERVISCOSITY SYNDROME with S/S of VISUAL IMPAIRMENT, NEUROLOGICAL PROBLEMS, BLEEDING ,CYROGLOBINEMIA.
one common feature anyway is both are progressive disesse with median survival of 2-4yrs.
