hey hedieh, nice to see someone posting on this thread after such a long time, may I ask u which country u belong to ? Where did u gain this 8 yr experience, I'm still in med school last year, so pretty much inexperienced, so hope to get some valuable advices from an experienced person like u, thanks smiling face

I was practicing in Iran, iranian medical education and work system is very competitive and demanding. I was working in a very crowded ER, in a 24 hour shift, I visited about 30 red & yellow traige patients & about 80 green triage patients. The hospital was located in subrub, in the margin of a busy freeway, with some factories close to it, so we had lots of car accident patients as well as occupational injuries( like electric burns, chemical poisoning, work-related accidents,...) as well as routine medical emergencies ( arrhytmia, stroke, MI,.......). At first, although the work was stressful, but it was exciting and interesting. After about 4 years, when almost all cases became repetative & there was nothing new for me, there was no excitation anymore, but the stresses of dealing with agitated patients and families and responsibility of patients remained.

I myslef am still in doubt if to apply for ER residency or not ( of course after I took the MLEs). On the one hand, I am tired of that stressful job, on the other hand, I like to use my experienceraised eyebrow.

well buddy, from my own experience in US er, I can tell at least one thing , it is much easier in US, u have at max 12 hr shifts and u work around 16-18 hrs a day, so it ain't that bad as compared to Iran

Correct me if I am wrong guys, but wasn't EM one of the specialties with the highest burn-out rates? I agree with young_doc that you work only 15-16 days a month...But at least half of those shifts are in the night. And the reality is (like in every other specialty, but more evident in EM) that you deal with a lot of weirdo patients, junkies, frequent flyers etc...A did several rotations in the US(not EM) but I did spend quite some time in the ER admitting patients. My impression was that ER docs become very cynical the older they get. And I am not blaming them- when you see what they are dealing with on a daily basis-they get yelled at, cursed at, threatened- it is damn hard to stay normal. Why do AMGs like it so much? Well, like everyone of us they have an ideal image in their heads- shift work, good salary, no call. But there is a another side too.


When I was n the us, I spent 10 weeks on trauma call. The first month was soooo exciting, the rush every time the alarm went off(varied between 5 to 20 times a NIGHT), it was exactly like in the ER show- 10 people surrounding the patient, intubating, IVs, drugs, X-rays, emergency surgeries...but afterwards it was like- what the hell: another drunken driver without seatbelt, feast fights, another drug dealer with a gun shot wound, suicide...After a while it gets old. At least for me. And guys, don't ever choose a specialty because of the money! Eevery one of them has advantages and disadvantages. The real question is: would you still be liking it in 15 years? Put everything in perspective. cardiologists make money, but depending on practice you may be on call very often and ready to do an emergency angioplasty(within 90 min after patien with MI comes in)Besides that most IMGs don't have to pay off any loans and you can easily live off 150k...So in my opinion- choose something that you still will be liking in 15 years, when you probably are going to have different priorities and maybe a family. Don't choose a specialty because it has easier/shorter residency. Most people regret it aftrewards.

Well, thats well said medicus_81. One should never choose a specialty based only on money, but the fact about ER is that it is not everybody's cuppa tea, u gotta have the guts to slug it out, so better be prepared for it
and no , u dun have to do the same thing all ur life, u can do a fellowship in Toxicology, sports medicine, or become a hospital administrator

regarding scores what is the cutoff score 4 ER residency programs??

there is no "cutoff" , but I guess both steps >250 is a good score

there seems to be a conflict in ur mind iranian, u seem to like EM, but still u r trying to convince urself that EM is not a good branch
