A febrile 5 yr old child ocmplains of chest pain. PE shows painful cervical adenopathy,dry cracked lips with erythema and swelling of hands and feet. A desquamating rash develops opn the fingers and toes.

Which of the following is a potential complication of the pt's disease

A. Acute MI
B. Aortic Arch Aneurysm
C. Aortic dissection
D. Infective Endocarditis

nice Q,
i feel it might be infective endocarditis..........
by...........staph aureus.........desq rash cos of the same organism.....S/S of SABE.........must not be rash its sub cut heamoorage........very common in this disease.....
i dont know the reason why there is lymphadenopathy???
not sure........i go with option D

will have to search and find it out......
whats the answer???

Kawasaki involving large/ medium/ small art----> complic thrombosis and Aneurysm formation or coronary art. ?

yup ! the Q smells kawasaki and MI when u start reading the PE ...

B may be the distractor bc Kawasaki results with CORONARY ARTERY aneurisms...

Prolonged fever, rash, mucocutaneous involvement, extremity changes, cervical adenopathy, conjunctivitis, and the development of coronary artery aneurysms characterize kawasaki

answer is A and B is a very good distractor

A Kawasaki
Typically, a child who is diagnosed with this illness will have a fever lasting 5 or more days and at least 4 of the following symptoms:

* redness in both eyes
* changes around the lips, tongue, or mouth
* changes in the fingers and toes, such as swelling, discoloration, or peeling
* a rash in the trunk or genital area
* a large swollen lymph node in the neck
* red, swollen palms of hands and soles of feet

the pt has Kawasaki Disease ( Mucocutaneous lymph node syndrome). Characteristic findings include painful Cervical lymph nodes, swelling of the hands n feet, a desquamating rash involving th efingers and vasculitis of the Coronary arteries.

B is incorrect as Kawasaki does not cause the Aortic Inflammation or of Arch of Aorta. Aneurysms r caused predominantly Atherosclerosis.

C,D and E are not associated with Kawasaki.
