A 46 y.o. female presents to the ER with fever, chest pain and hemoptysis.She came to visit her family from Pakistan where she works in a goat wool processing center. Chest X ray shows widened mediastinum.Sputum and blood cultures reveal large gram positive rods that form "medusa head" colonies on standard media.
The bacteria isolated from this patient most likely produce:

a) Antiphagocytic D glutamate capsule
b) Antiphagocytic polysaccharide capsule
c) Ig-G Binding outer membrane protein
d)Intracellular polyphosphate granules
e) Peritrichous flagella

medusa head colonies...thats a big diagnostic point of bacilus anthracis' colonies....which contain poly D-glutamate capsule...so answer is A

hemorrhagic mediastinitis seen as WIDENING OF MEDIASTINAL SHADOW

caused pulmonary anthrax...

D-glutamate...only medical significant bacteria with polypeptide capsule...

others non polysaccharide capsules of significance are

hyaluronice acid.... stept pyogenes...
sialic acid....meningococcus type b ... weakly immunogenic so cant be used for making vaccines
chlamydia have altered LPS ...used to detect them by complement fixation...

I didn't see this q in my UW. I just finished current updated UW 2000Q.
Is there any different version?
