I invite everybody intrested in serious discussions and cross questioning. I intend to write CK in the first week of september and have finished first reading of KAPLAN + various Q-banks, now starting to regroup myself with exam perspective....writing all that i still can not understand and recalling important clinical perls.
Intend to do system wise - med.-paed.-surgery-pearls-questions
Starting from medicine- GI, join me on my way towards building a solid foundation....we all can do it !

Esophagitis-candida,HSV,CMV,Varicella,HIV - know medicine for all of them as seperate entity.
in HIV- think of candida (cottage cheese flake) first -initiate fluconazole then see response, if not- pridnisone
Barretts - if no dysplasia-EGD every 2-5 years , low grade-3-6 mths, high grade-resection
Upper 1/3 - Scamous cell Ca- "Sca"- hypercalcemia
Lower - barrets- adenocarcinoma
Ba swallow- apple core - Ca
Zenker div.- Ba swallow - NO EGD and intubation-risk-perforation
Lt.supraclavicular lymph node - wircow node-esophageal Ca
Lymph node around umblicus- sister mary joseph- colon Ca
